§ 25¹. Õpilase tervisekaardi lisalehe kanded
(1) Õpilase tervisekaardi lisalehele vormistatakse õendusplaan juhul, kui õpilasel tuvastatakse tervisealased kõrvalekalded. (2) Õpilase tervisekaardi lisalehele kantakse õendusplaani järgmised andmed: 1) kande tegemise kuupäev; 2) õendusdiagnoos; 3) õendusabi eesmärk; 4) planeeritud õendustegevused; 5) teostatud õendustegevused; 6) hinnang; 7) tervishoiutöötaja ees- ja perekonnanimi, registreerimiskood ning allkiri.
